1. Only complete this form if your child is transferring school during the school year or if you are applying late for entry in to First/Primary School (Reception), Middle School (Year 5), Secondary School (Year 7) and High School (Year 9) in September 2022.
   If you are applying for a nursery place, you must contact the provider directly.
2. This application form should only be completed if you are resident in Northumberland or in the process of moving into a Northumberland Address.
3. Only one form should be completed for each child.
4. This form should be completed even if your child has a statement of Special Educational Needs or an Education Health and Care Plan (EHCP), or is currently undergoing a Statutory Assessment
